Background: Pediatric residency is characterized by high emotional and cognitive demands, with frequent exposure to difficult clinical events. Wellness resources and intentional breaks are essential for supporting resident resilience and mitigating burnout. Evidence-based interventions, including movement, meditation, nutrition, music, games, and social engagement, have been shown to buffer stress, enhance adaptive coping, and foster psychological safety. Providing easy access to a repository of wellness resources can facilitate use of these interventions during busy clinical shifts and after distressing events.

Objective: We aim to decrease the rate of pediatric resident burnout from a baseline of 60% to 25% over a 5-month period.

Design/Methods: This quality improvement (QI) project was conducted using the Model for Improvement to increase pediatric resident well-being by promoting use of accessible wellness resources. The intervention was implemented over a 5-month period (September 2025-January 2026) within a single academic pediatric residency program and led by a team of two pediatric hospitalists and one pediatric resident champion. All pediatric and internal medicine-pediatric residents (PGY-1 to PGY-4) were included. Data were collected during required Pediatric Residency Academic Half-Day (AHD) sessions every two weeks, and baseline data was collected over two months.

Our primary Outcome Measure is the percentage of residents scoring ≥4 on the Resident Well-Being Index (RWBI), which indicates burnout, collected biweekly. Our Process Measures include both the RWBI completion rate and percentage of residents reporting use of ≥1 wellness resource in the prior two weeks. Our Balancing Measure is the percentage of residents reporting that the intervention added time burden and/or interfered with clinical/personal responsibilities, measured via a 5-point Likert scale biweekly.

Our interventions and PDSA cycles began in November 2025 with cycle 1 targeting the creation of an online repository of wellness resources organized around five domains ("5 Ms"): Mindfulness, Music, Movement, Mindplay, and Meditation. Cycle 2 will begin in mid-to-late November with the distribution of printed "badge buddies" with a QR code linking directly to the online resource collection. Cycle 3 will occur in December with reorganization and refinement of the resource platform based on resident feedback from brief surveys and open-ended comments.

To analyze our data, run charts will track outcome and process measures over time, with special cause variation identified using established QI rules.
