Background: Fostering independence and healthy habits is key to a strong developmental trajectory. Adolescents, including those with developmental conditions, may find the transition to autonomy difficult, placing significant responsibility on caretakers for guidance and consistent reminders. As 95% of teenagers have cell phone access, mobile apps may serve as accessible tools to reinforce daily routines and promote mental and physical health without parental involvement. Many of these apps employ operant-conditioning strategies (OCS) to encourage healthy habits. Identifying apps for independence and assessing user receptivity to conditioning strategies will inform caretakers and clinicians about effective independence-building health interventions.

Objective: To examine independence apps for use in adolescents; to identify effective OCS (positive reinforcement (PR), negative reinforcement (NR), positive punishment (PP), and negative punishment (NP)) in independence apps.

Design/Methods: Popular independence apps (n=50) from 5 categories (habit trackers, health trackers, period trackers, focus apps, and study apps) were analyzed. App store rating and OCS (PR, NR, PP, NP) used in each app were recorded (Figure 1). Association between app rating and OCS type was assessed with a chi-square test. 3 independent raters used an 8-item rubric (1-4 scale) to evaluate independence-building features, customization, motivational feedback, integration into daily life, and reliance on parent-involvement/reminders (Figure 2). The highest scoring app in each category was calculated.

Results: The research team compiled a detailed descriptive analysis of how each app category supports pediatric independence (Figure 3); the team also categorized in-app features by OCS type (Figure 1). Across app types, PR strategies were used most frequently (78%). High user ratings (4.7-5 stars) were associated with higher use of PR (p=0.011). Total rubric score was highest for the following apps in each category: Finch (habit), Lifesum Food Tracker (health), Flo Cycle & Period Tracker (period), Forest: Focus for Productivity (focus), and Khan Academy (study) (Figure 3).

Conclusion(s): Independence apps can help foster autonomy in adolescents, transferring the onus from caretaker to child. The association between PR use and higher user ratings suggests that adolescents are more motivated by encouraging feedback than by punitive design features. These findings may be applied to other independence-building interventions to reach a younger demographic. Clinicians should consider recommending independence apps, particularly those that utilize PR.
